These forms can be used in our first four sessions and with the exception of the contract, can be downloaded below. All forms will be emailed to me at tim@faithfullygrowing.com.

Please email me your signed contract prior to our first session. I'll send you the contract and you will email the completed contract to me.

Please fill out this form before your first coaching session or group meeting and email it to me.
2. The First Step De-Cluttering Form
Please complete and email this form before our first coaching session or group meeting.
3. Session Preparation Form
Please fill out this form before each coaching session or group meeting after your first session or meeting. The Session Prep Form should be emailed to me at least 24 hours prior to each coaching session or group meeting to help me better prepare for our time together.
4. Goals Form
Please complete this form before our second coaching session or group meeting and email it to me.
5. Your Life Story Form
Please complete this form before our second coaching session or group meeting and email it to me.
6. Your Wheel of Life Worksheet
Please complete and email me this form before our third coaching session or group meeting.
